| Colistimethate sodium is a chemically modified prodrug of colistin (polymyxin E). Methanesulfonation neutralizes free amino groups, significantly reducing direct nephrotoxicity and neurotoxicity while greatly improving water solubility. It slowly hydrolyzes in vivo to release active colistin, which acts on Gram-negative bacterial outer membrane LPS and phospholipids, disrupts membrane integrity and leads to bacterial lysis. It exhibits potent bactericidal activity against multidrug-resistant Gram-negative bacteria such as Pseudomonas aeruginosa and Acinetobacter baumannii.
